1. Payment Information: Valid credit card number (MasterCard, Visa, American Express, Discover, Diners Club), cardholder name and billing address.
2. Business Information: Your Federal Tax ID number or, if you don't have one, your Social Security number (Business Number or Social Insurance number for Canadian residents).
3. State/Province Resale Tax Certificate: You will be asked to submit a copy of your Resale Certificate or Uniform Use & Sales Tax Certificate.
4. Owner Information: name, address, phone number and email address.
